PHASE I: Services included during this phase:

  1. Analysis of consortium needs and programs eligible for filing for E-Rate discounted services for each Funding Year.
  2. Completion of All forms and requirement to receive funding related to each Funding Year.
  3. Verification of Discount Rates for individual schools.
  4. Development of the Form 470 Application for services to be verified by the school prior to submission.

PHASE II: Services included during this phase:

  1. Filing of the Form 470 for all requested services.
  2. Maintain written records of all required documentation.
  3. Respond to vendor inquires and bids.
  4. Develop reports and bid documents for action.

PHASE III: Services included during this phase:

  1. Filing of the Form 471 for all requested services.
  2. Maintain written records of all required documentation.

PHASE IV: Services included during this phase:

  1. Filing of the Form 486 for all funded services.
  2. Filing of the Form 472 for reimbursements from approved vendors.
  3. Respond to all USAC SLD requests and questions.
  4. Provide written documentation for required five (5) year documentation files.

On-Going Work: Services included:

  1. Review of the development of RFP’s.
  2. Update the schools on programs rules and regulations, as needed.
  3. Respond to school needs, as required.
  4. Respond to all requests from the USAC Schools & Library personnel.
  5. Provide professional and technical guidance to the consortium, as requested within the scope of the E-Rate identified phases.

Special Circumstances Related to Previous Filings: FPC, Inc. will not be responsible for Program Compliance rule violations on previous E-Rate filing, if discovered during the course of review of previous filings.

Initial Proposals do not include any after funding audits or the 30 day Selective Review, unless specifically agreed to in the initial contract.
